GSA Millennium Award for Community Service

Deadline to apply: October 15, 2021, 4 p.m. ET

The application consists of:

  • webform
  • one-page summary of the applicant’s volunteer history
  • one or two-page reflection by the applicant
  • two (2) one-page supporting letters from volunteer organizations

This form includes text, dropdown and multiple choice fields. All fields are required.

Volunteer Summary

Please provide a one-page summary of your volunteer activities for the past five years, including the length of time spent with each activity.

File should be named as MacGrad_Summary2021.docx and uploaded to our Millennium Award MacDrive.

Reflection Statement

Please provide a one or two-page double-spaced letter with 1-inch margins and 12pt font describing why you chose to volunteer in any of the listed activities, and what you have gained from the experience(s). File should be named as MacGrad_Reflection2021.docx and uploaded to the Millennium Award MacDrive.

Confidential Supporting Letters

Supporting letters may be a maximum of one page in length and should indicate the length of time involved with the organization as a volunteer and any comments.

Letters from referees must be uploaded to our Millennium Award MacDrive folder. Files should include the name of the applicant, e.g., MacGrad_Millennium2021.docx.
